Established in 1991 amid Yugoslavia's turmoil, Division 1 truly flourished post-Kosovo's 2008 independence. UEFA and FIFA recognition in 2016 marked a turning point, elevating standards. The 2018/19 season shone when Ballistiku from Gjilan shocked favorites to earn promotion via playoffs. Notably, players like Valon Berisha transitioned from here to Kosovo's national team during Euro 2020 qualifiers.

Tournament format

Comprising 12 teams, the league follows a double round-robin format—22 matches per side. The champion ascends directly to Superliga; the runner-up contests playoffs against the 10th-placed elite team. Since 2020, fair play rules and a four-foreigner limit apply. The season spans August to May, with a winter break.
