History and foundation
Founded in 1992 post-Yugoslav independence, the league saw Vardar Skopje dominate early with seven straight titles from 1993-1999. Shkëndija rose in the 2000s, clinching 17 championships by 2024, making it the most successful club. Notably, in 2017, Vardar reached the Champions League group stage after knocking out Fenerbahçe and AEK Athens. The league faced corruption issues in the 2010s but reforms by the Football Federation of Macedonia revitalized it.
Tournament format
The format is a double round-robin: 12 teams play 33 matches each. Top three earn European qualification, while 11th and 12th face relegation or playoffs with second-division sides. Recent seasons introduced a split system: top six and bottom six play extra fixtures post-regular season.
