Launched in 2021 as part of FIGC's youth league overhaul, Primavera 4 replaced the former Primavera 2 structure to boost competitiveness at the third tier. Monza claimed the inaugural title, followed by standout campaigns from Catanzaro and Virtus Francavilla. Notably, talents like Matteo Cestana (now at Juventus) have graduated from here. The reform introduced promotion playoffs to Primavera 2, injecting drama—witnessed in epic finals that have drawn thousands of fans.

Tournament format

The competition splits into 9 regional groups of 14–16 teams each, playing a double round-robin (26–30 matches per team). Group winners and top runners-up advance to playoffs: first for promotion spots to Primavera 2, then a national phase culminating in a grand final. The season runs from September to June, integrating cup ties for added depth.
